Anavar Side Effects: Men, Women, Blood Work and Reversibility
Why "Mild" Is a Relative Term
Anavar side effects are the reason the compound has the reputation it has, in both directions. Oxandrolone is the mildest oral anabolic steroid in common use, the least androgenic, the only one that does not aromatise, the gentlest 17-alpha-alkylated oral on the liver, and every one of those statements is a comparison, not an absolute. Measured against Dianabol or Anadrol it is mild. Measured against nothing, it raises liver enzymes in nearly every user, cuts HDL cholesterol by a third to a half, suppresses natural testosterone, and can virilize women. The side effects of Anavar are real; they are simply smaller and more predictable than the alternatives'.
This page is the hub for the whole subject. It sets out the effects by body system with the frequencies and magnitudes the blood work shows, the specific profiles for men and for women, what oxandrolone does not cause and why, the long-term picture, the benefits and side effects side by side, and how dose, length and monitoring reduce the risk. The detailed guides on the liver, hair loss and women go deeper on each; the side effects overview page summarises the basics.
The four Anavar side effects that show up in almost every user are: ALT and AST at 2 to 3× baseline, HDL down 30 to 50%, SHBG down 40 to 60%, and testosterone suppressed 30 to 50% by week four. Everything else, hair, acne, mood, virilization, depends on dose, length and who is taking it.
Side Effects of Anavar by System
The clearest way to see the side effects of Anavar is by system, with how often each appears and whether it reverses.

The pattern is that the measurable effects, liver, lipids, hormones, are near-universal but reversible, while the visible effects, hair, acne, mood, are individual and dose-dependent. What does Anavar do to your body over a standard cycle is mostly the top three rows; what a given user notices is usually one of the lower ones.
Anavar Side Effects in Men
Anavar side effects in men centre on four things: the liver, the lipids, suppression, and, for the predisposed, hair. The liver effect is the one the compound is famous for being gentle on, and it is: ALT and AST rise two to three times baseline over a six-week cycle at 40 to 50 mg, against three to five times on Winstrol and five to ten on Anadrol, and twelve-week clinical courses at 20 mg have not impaired liver function. The liver guide covers the enzyme tables and what NAC and TUDCA do about them.
The lipid effect is the one it is not gentle on. Oxandrolone cuts HDL by 30 to 50% within four weeks, one of the larger drops of any oral, with a modest rise in LDL, a change that matters more the longer the cycle and the older the user. Suppression is covered in its own guides: total testosterone falls 30 to 50% by week four on a solo run, which is why the Anavar only cycle feels flat in its second half and why Anavar PCT is not optional for men. Hair is the fourth: oxandrolone is a DHT derivative that acts at the follicle, and men with a family history of male-pattern loss report accelerated thinning at 50 mg and above, as the Anavar and hair loss guide sets out. The anavar side effects male users worry about most, gynecomastia and water, are the ones it does not cause.
Anavar Side Effects in Women
Anavar side effects women experience are different in kind, because the risk that matters is virilization rather than suppression. At 5 to 10 mg for six weeks the reports are of acne, oiliness, a rise in libido and clitoral sensitivity, and, in a minority, menstrual irregularity that resolves within four to eight weeks of stopping. At 15 to 20 mg, or beyond eight weeks, the androgenic signs appear: voice deepening, clitoral enlargement, facial and body hair. Voice change is the one that becomes permanent if use continues after it starts, which is why the female protocols are built around stopping at the first sign rather than the third.
The liver and lipid effects apply to women at proportionally smaller magnitude, ALT and AST 1.5 to 2× at 10 mg, HDL down 20 to 30%, and women do not run SERM-based PCT. The Anavar side effects in women guide has the thresholds and the stop rules, and the Anavar for women guide has the protocol that keeps most users below them.
"A man's strength lies in his ability to stay standing when it would be easier to fall.", Yujiro Hanma
The side effects that end badly are almost always the ones a user decided to push through. Standing, in this context, means stopping.
What Anavar Does Not Cause
Three things people expect from an anabolic steroid do not happen on oxandrolone, and the reasons are structural. It does not aromatise, the A-ring modification that makes it oxandrolone rather than DHT removes the substrate the aromatase enzyme needs, so there is no conversion to estradiol. That means no estrogen-driven water retention, no bloat, and no gynecomastia from the compound itself; does Anavar cause gyno is a question with a clear no, with one caveat covered below. It also does not convert to DHT, because it already is a DHT derivative, so the DHT-specific effects of testosterone, prostate stimulation, some of the hair effect, are lower than testosterone's even though the molecule acts at the same receptor.
The caveat on gynecomastia is the post-cycle window after a solo run: with testosterone suppressed and nothing replacing it, estrogen can drift proportionally high for a few weeks, and a susceptible user can develop soft tissue then. That is a suppression effect, not an aromatisation effect, and PCT prevents it. Does Anavar cause water retention has the same answer, not from the compound, though a user who gains five pounds of water in week two is very likely taking Dianabol sold as Anavar, a substitution the Anavar for sale guide explains.
Long-Term Side Effects
Anavar long term side effects are less studied than the acute ones, because the clinical courses ran twelve weeks or less and recreational users cycle. What the evidence supports: repeated cycles without full lipid recovery between them accumulate cardiovascular risk, and the HDL suppression is the mechanism; repeated liver enzyme elevations without recovery are associated with peliosis hepatis and, rarely, liver tumours in the 17-alpha-alkylated class as a whole, though oxandrolone's record is the cleanest in that class; and repeated suppression cycles can lengthen recovery time with each cycle, particularly in users over 35. Virilization in women is the one long-term effect that is definitively permanent once past a threshold.
What the evidence does not support is the idea that a handful of six-week cycles with full recovery between them produces lasting harm in a healthy adult; the cases in the literature are almost all long-duration, high-dose or stacked. Is Anavar bad for you is therefore a question about pattern rather than compound: the same 50 mg is a manageable six weeks or a damaging year depending on what surrounds it.

The Anavar before and after guide shows the left column; this page is the right column, and the two are not separable. Anavar pros and cons discussions that list only one side are describing a different compound.
Reducing the Risk: Dose, Length and Blood Work
Three variables control almost all of the risk. Dose: 40 to 50 mg for men and 5 to 10 mg for women are the points where results are near maximal and blood work is still manageable; the Anavar dosage guide shows how fast the liver and HDL cost rises above them. Length: six weeks, eight at most, because results plateau while enzymes and suppression keep climbing, the Anavar cycle guide sets out the structure. Blood work: a liver panel, a lipid panel and a hormone panel before the cycle, at week four, and six to eight weeks after PCT, which turns every side effect on this page from a surprise into a number that can be acted on.
Around those three sit the supports: NAC 1,200 mg or TUDCA 500 mg daily for the liver, zero alcohol on cycle, a testosterone base for men who want to avoid the late-cycle low, PCT planned before the first tablet, and the Anavar results timeline as the map of what should be happening at each checkpoint. The Anavar Reddit reviews guide is a fair record of what happens to users who skip them.
Frequently Asked Questions
In nearly every user: liver enzymes at two to three times baseline, HDL cholesterol down 30 to 50%, SHBG down 40 to 60% and natural testosterone suppressed 30 to 50% by week four. In some users: acne, painful pumps, mood and sleep changes, accelerated hair thinning if predisposed. In women above 10 mg: virilization. Most reverse within weeks of stopping.
It carries real, measurable costs, liver, lipids, suppression, that are smaller than other oral steroids' and reversible at normal cycle lengths with recovery between cycles. The harm in the literature comes from long, high-dose or stacked use without recovery. The same 50 mg is a manageable six weeks or a damaging year depending on the pattern around it.
Not directly. Oxandrolone does not aromatise, so it produces no estradiol and no estrogen-driven breast tissue. The one route to gynecomastia is the post-cycle window after a solo run, when suppressed testosterone leaves estrogen proportionally high for a few weeks in susceptible users, a suppression effect that PCT prevents.
Modestly in a minority of users, 5 to 10 mmHg, through lipid changes and, at higher doses, slight fluid shifts, but without the estrogen-driven water retention that pushes blood pressure up on aromatising compounds. Users with existing hypertension should monitor it; for most it returns to baseline within weeks of the cycle ending.
The measurable ones are: liver enzymes normalise in four to eight weeks, HDL in six to ten, and testosterone with PCT. Acne, pumps and mood resolve on stopping. Two are not fully reversible: hair loss in predisposed men, which reverses only partly, and virilization in women, voice change in particular, once use continues past the first sign.
It is the mildest oral in common use: least androgenic, non-aromatising, the gentlest 17-alpha-alkylated compound on the liver, and the only one with extensive clinical use in women. "Safest" is still relative, it cuts HDL by up to half and suppresses testosterone, so it is the lowest-risk option, not a risk-free one.
