Anavar Benefits and Pros and Cons: What the Trials Measured
What the Anavar Benefits Actually Are
The Anavar benefits that have numbers behind them come from a small set of clinical trials, and they are more modest than the forum version. In the best-known of them, Schroeder and colleagues (Journal of Applied Physiology, 2004) gave 32 men aged 60 to 87 either 20 mg of oxandrolone a day or placebo for 12 weeks, with no training programme. Lean body mass rose by 3.0 kg, thigh muscle area by 12.4 cm², strength on the leg press by 6.7% and on the chest press by 9.3%, and total fat fell by 1.9 kg, all significantly more than placebo. Those are the measured benefits of Anavar in men: lean mass, strength and fat, at a clinical dose, without a gym.
Everything users add on top, dryness, hardness, vascularity, the "no water" look, the mildness in women, comes from the drug's chemistry rather than from a trial, and most of it holds up as mechanism. This article lists the benefits in order of how well they are supported, then sets the cons against them, because every one of the pros has a cost attached.
Key takeaway: the proven benefits are about 3 kg of lean mass, 5 to 9% more strength and about 2 kg less fat over 12 weeks at 20 mg, with no aromatisation and no water. The cons are measured in the same trials: HDL down, LDL and liver enzymes up, and the body's own testosterone suppressed.
Benefit 1: Lean Mass Without Water
Oxandrolone is a DHT derivative modified so that it cannot be converted to estrogen, so none of the weight it adds is estrogen-driven water. In the Schroeder trial the 3.0 kg of lean mass came with a 2.9 kg rise in total body water, which is the water inside new muscle, not under the skin. Users describe the result as "dry" gains that look like muscle in the mirror, and the mechanism supports that. The size of the gain is smaller than testosterone or Dianabol produce, which is the trade covered in Anavar vs Dianabol and Anavar vs testosterone.
Benefit 2: Strength
The same trial measured one-repetition maximums: leg press up 6.7%, leg flexion 7.0%, chest press 9.3%, lat pull-down 5.1%, in untrained men in their sixties to eighties who did not lift during the study. Users on 40 to 50 mg who train report larger numbers, commonly a 10 to 20% rise on compound lifts over six weeks; those figures are forum reports, not trial data, and the clinical range is the floor. Strength arrives before size, usually from week two, which is why the results week by week guide puts it first.
Benefit 3: Fat Loss, Including Visceral Fat
This is the benefit with the most surprising evidence. In the companion paper (Schroeder, JCEM 2004) the same 20 mg dose reduced total fat by 1.8 kg, trunk fat by 1.2 kg and, on MRI, visceral abdominal fat by 20.9 cm², with a measurable improvement in an insulin-sensitivity index. Twelve weeks after the drug was stopped, most of the fat loss was still there, while the muscle and strength gains had returned to baseline. The mechanism and the limits are in does Anavar burn fat.
Benefit 4: No Aromatisation
Because oxandrolone does not convert to estradiol, the estrogen side effects of testosterone and Dianabol, bloating, gynecomastia from the compound itself, blood-pressure rises from water, do not occur on Anavar alone. That is the structural reason it is the oral chosen for cutting and for women, and the reason it needs no aromatase inhibitor when run on its own. The one exception, a post-cycle estrogen drift after a solo run, is a suppression effect covered in Anavar side effects.
Benefit 5: The Lowest Virilisation Risk Among Orals
Oxandrolone was designed to separate anabolic from androgenic activity, and the FDA label acknowledges the attempt while noting that complete separation "has not been achieved". In practice it is the only oral anabolic steroid with a history of use in women at 5 to 10 mg, which is a relative benefit, not an absence of risk; Anavar for women sets out the stop rules.
Benefit 6: An Oral With a Clinical History
Oxandrolone was prescribed in the United States from 1964 until its approval was withdrawn in 2023 (for commercial, not safety, reasons), for weight gain after surgery, trauma and chronic infection, for bone pain in osteoporosis, and in practice as a standard of care in burn recovery. Twelve-week courses at 20 mg in clinical settings did not impair liver function, which is the basis for its reputation as the gentlest 17-alpha-alkylated oral. The clinical uses and the current legal position are in Anavar prescription and legality.
The Cons, From the Same Trials
Anavar pros and cons, with the source for each
+3.0 kg lean mass, +5 to 9% strength
Schroeder 2004, 20 mg, 12 weeks
Gains gone 12 weeks after stopping without training
Same trial, week 24
−1.8 to −1.9 kg fat, visceral fat down
Schroeder 2004 (JCEM)
Modest in absolute terms; diet still does the work
Same trial
No water, no aromatisation
Chemistry; FDA label
Post-cycle estrogen drift after solo runs
User reports; side-effects guide
Mildest 17-alpha-alkylated oral
Clinical courses at 20 mg
Still raises ALT/AST and LDL, lowers HDL
Grunfeld 2006 (20 to 80 mg)
Lowest virilisation risk of the orals
Label; clinical use in women
Risk is reduced, not absent
Label
Oral, no injections
Form
Suppresses SHBG, LH, FSH and own testosterone
Grunfeld 2006
The Grunfeld trial (JAIDS 2006, 262 men with HIV-associated weight loss, 20, 40 or 80 mg for 12 weeks) is the one that quantifies the cost: oxandrolone significantly suppressed SHBG, LH, FSH and total and free testosterone, and raised transaminases and LDL while lowering HDL, at every dose. That is why the cycle guide caps a course at six to eight weeks and why Anavar PCT exists. The question of whether the trade is acceptable is answered person by person in is Anavar safe.
Who Gets the Most From It
The benefit profile favours three users: the lean trainee in a deficit who wants to hold muscle and strength while losing fat, the woman who wants an oral with the lowest androgenic signal available, and the man on a testosterone base who wants dryness in the final weeks of a cut (Anavar and TRT covers the replacement-dose version). It serves a bulk badly: the lean-mass gain is small for the liver cost, and the compounds built for mass are compared in the stacking guide.
In the clinical trial at 20 mg a day for 12 weeks: about 3 kg more lean mass, 5 to 9% more strength and about 2 kg less fat, including visceral fat. Users add dryness and hardness, which follow from the drug not aromatising.
It adds lean mass and strength without water, reduces fat, and does not convert to estrogen. It also suppresses natural testosterone and worsens cholesterol, which is why men run it on a testosterone base for a limited number of weeks.
For cutting while holding muscle, and for women who want the mildest oral, the benefits are real and measured. For bulking the lean-mass gain is small for the liver and lipid cost; testosterone or Dianabol do that job better.
Yes, modestly: 3.0 kg of lean mass in 12 weeks at 20 mg in untrained older men, with thigh muscle area up 12.4 cm². The gain disappeared 12 weeks after stopping without training.
Suppressed testosterone, LH, FSH and SHBG; raised liver enzymes and LDL; lowered HDL, all measured in a 262-man trial at 20 to 80 mg. Virilisation risk in women is reduced, not absent, and the gains fade after stopping.
